If you’re an older expat or planning to retire in the Philippines, Thailand, or elsewhere in Southeast Asia, health insurance is one of the most important — and most dangerous — decisions you’ll make. Many newcomers go for the cheapest local policy because the price looks attractive and the brochure sounds impressive. That decision often turns into an expensive nightmare when they actually need to claim.

1. How the Premium Is Really Calculated

Most local policies use individual rating. Stay healthy and your premium stays low. Develop a serious condition — cancer, heart disease, or anything major — and your premium can skyrocket at renewal, or worse, the insurer adds exclusions or refuses to renew.

I prefer pooled-risk policies where your risk is shared across a large group. This gives far better long-term stability, especially important when you’re 70, 75, or 80 and most vulnerable. Health insurance isn’t about this year — it’s about whether the policy will still protect you in the future.

2. Who Holds the Insurance Company Accountable?

Jurisdiction matters more than most people realise. If a claim is denied unfairly, what real options do you have?

Local policies in some Southeast Asian countries offer limited consumer protection. I prioritise insurers based in highly regulated environments (like the UK) that give you access to independent ombudsman services and strong dispute resolution. When you’re facing a six-figure medical bill, you need more than promises — you need real accountability.

3. What Isn’t Covered? (The Hidden Gaps)

High hospital limits look great on paper, but check the fine print:

  • Outpatient treatment
  • Specialist consultations
  • Advanced diagnostics
  • Emergency medical evacuation

Living in a remote area of the Philippines or Thailand, a serious emergency may require evacuation to Singapore, Bangkok, or Manila. Many cheap policies fall short exactly when you need them most.
